Tennessee Supports and Services

Encare is licensed and contracted by the Tennessee Department of Intellectual and Developmental Disabilities (DIDD) under the Medicaid Waiver. We provide a variety of services including Supported Living, Personal Assistance, Community Based Day, and Supported Employment. Encare, LLC’s office is located in Jackson, TN.

Supported Living

We offer supports and services in a community environment. We assist the person supported to choose a home in the community of their choice. One of our home managers will assist the supported person with managing their home.   Supported living services are designed and implemented to accommodate the choices, abilities and needs of each person and provides the supports necessary to enable a person to reside in a home that is under his/her control and responsibility. Services include transportation.

Personal Assistance

Encare offers personal assistance services for a person who is living in the home of his/her family member or non-related caregiver. Personal Assistance services include assistance with daily living activities and community access.

Community-Based Day Service

Community-based day services provide opportunities that enable a person to participate in meaningful, productive, and life-enhancing community activities of his/her choice.

Supported Employment Services

We provide employment evaluations and job placements for a person seeking employment. Through coordination, planning and community linkages, Encare will provide goal-oriented and systematic employment services and supports. Encare will work in cooperation with the Tennessee Department of Vocational Rehabilitation Services to provide employment services.
